have issued orders in the G.Os 1st and 2nd cited modifying the
format of Annual Confidential Reports of Non Gazetted Officers / Gazetted
Officers. Further Orders were Issued in the G.O. 3rd Cited adding a
column to the modified Annual Confidential Reports to intimate the date of
submission of Annual Property Returns Statement every year after column 15 in
Form -A, Part-II, as 15 (a) in respect of non-Gazetted officers and after
column (10) in form 'A'., Part-II as 10 (a) in respect of Gazetted Officers.
2. In the reference 4th cited, the
Director General, Anti-Corruption Bureau has stated that it has been noticed
that certain departments are still using old formats of the Annual Confidential
Reports, and requested to issue suitable instructions to all the Departments to
strictly follow the · revised format of Annual Confidential Reports, as per
orders issued in the G.O. 3rd cited.
3. Therefore, all the Departments of Secretariat,
Heads of Departments and District Collectors are requested to strictly follow
the format of Annual Confidential Reports modified in G.Os 1st and 2nd
cited read with G.O. 3rd cited and bring it to the notice of all the
concerned for strict implementation of the above instructions. (Format of Form
A Part-II in respect of Non Gazetted Officers / Gazetted Officers are enclosed
for ready reference)
